Need: examination. Depot provera (medroxyprogesterone) is known for abnormal bleeding. It sounds like uterine spasms to me. This can radiate to many places.
But, you also need to worry about appendicitis, bladder infections, pregnancy to name a few. Your doctor can determine more precisely what your problem is. If it is too bad, go to an urgent care or ER

Call your GYN: If you are having significant pain, you probably need to see your GYN to make sure you don't have an infection. It could also be a ruptured cyst.
