Depends: If you had any type of trauma to the region, you should go immediately. If no trauma you could have an infection. Pain in that region usually warrants a trip to the ER especially if it is continuous.

Family doctor: No. An infected varicocele or epididymis or other scrotal structure is a common problem your primary care physician can manage. Go see your family doctor first thing in the morning.
