Are you asking this-: For a male person? It can be an ischial bursitis, if its off on one side (ie, not in the center). Is there any hx of a fall (like doing the splits)? It could be some inflammation from an infection there. Without an examination, its difficult to see what's happening? If its getting worse, then he should see his pcp.
