Ovulation: Women with regular cycles, have a surge in lh hormone levels that matures and ovulates (releases) the egg 24 - 48 hours later. This is the time you can get pregnant. If you have intercourse after 2-3 days ovulation, the chance of conceiving is far less but not zero. After 5 days, the chance is < 1% unless ovulation occurred later than you anticipated.

Yes and no: Conception can only occur if the women ovulates and is more likely to occur if intercourse occurs closer to the time of ovulation. However, a women that has sex one time six days before ovulation has about a 3 out of 1000 chance of conceiving. Her chances are about 1 out of 10 if she has sex near the time she ovulates. By about 24 hours after ovulation it is too late to have sex and conceive.

Common: Many women menstruate but do not ovulate. It is often from hormone level issues. Should discuss with your gyn regarding fertility testing to check hormone levels as "simple" place to start.

Depends : Depends on multiple factors. Age, general health, length of time taking pills, etc. I've had patients become pregnant 2 weeks after stopping pill. Others take many months. Everything being equal 4to6 months.
