See doctor: He might need testing to see the size of his colon or a rectal exam to evaluate the sphincter. If it is habit constipation and encopresis only, then the doctor can tell you how to treat it. It is usually a long term problem and treatment.

More information: Does he poop regularly? Does he go many days between poops? Does he have abdominal distension? Is his appetite affected? Sounds like he needs to see a pediatric gastroenterologist or pediatric surgeon.
