Tricky, but: There are several signs including depression, withdrawal from friends or family, bad breath, smooth fingernails or thin skin on back of finger. The dentist can also notice signs to the back of teeth that show thinning of enamel from stomach acid.

Number of signs: Someone who eats a lot then will excuse themselves and spend time in bathroom. Receipts for laxatives. Sometimes their dentist will notice acid erosion on teeth, from induced vomiting. Anorexia, avoidance of food, can go hand in hand, but is different.
