It is possible but: This is not a typical symptom from uterine cancer which often causes irregular vaginal bleeding and or pelvic or back pain.

Yes and depends: Doesn't always mean spread but local progression and inflammation can cause cramping with back pain, with radiation to the groin & also low back/sacrum & even pain radiation to knee.
