Epididymitis: Signs/sxs appear to be epididymitis of the testis, or orchitis. You need a scrotal ultrasound and possibly antibiotics. An incarcerated inguinal hernia, beyond pain and a knot, will eventually cause a bowel obstruction, which doesn't sound like your issue.

Possible: Could be muscle strain, but doesn't sound like hernia. What i would really like you to do is see your pcp and get the area checked for testicular infection or testicular cancer, the knot could be a lymph node.
