Piercing or trauma: A hole can be interpreted several ways. It could be a small ulcer, which could look like a small crater. These are usually pretty sore and tender. May be red with a whitish crust. It could be an actual hole where there is communication between the inside of the mouth to the outside, but this would not be very likely unless there was trauma or a piercing. Best to see your doctor to help you.
