Tubal failure: If a pregnancy test was positive then your tubal did fail. Sometimes the cut ends of the tubes are close and heal together. You need to use another method of birth control. If a woman gets pregnant after a tubal half the time the pregnancy is in the tube - an ectopic pregnancy - a risky situation. You need to speak to your doc about what to do now.
