When does a bilirubin level become worrisome?

Depends on the : Clinical context. In an adult al level above 2.0 mg/dl needs to be looked into to establish a cause, though the level by itself is not dangerous. In a five day old infant a level as high as 5 mg/dl would not be a cause for concern.
