Timing of boob job.: It is best to delay augmentation until after you have kids for several reasons. First, many women loose volume after having children or occasionally gain it, so the implant could wind up the wrong size. Also some get a lot of drooping and may need a lift as well as an implant. Finally, you might not be able to breastfeed after implants, especially if the incision was through the nipples.

Implant nursing: There is no one correct answer. If you are planning on having children soon, i would advise to defer implants because it is possible you will need another procedure such as a lift. However if it is in distant future you could consider implants now. Some would advise avoiding the areolar incision to minimize duct injury. Some have suggested that nursing may lead to a higher rates of contracture.

Waiting is best: Pregnancy often drastically changes the shape and size of the breast so waiting can often make sense. That way, any undesirable changes in breast size and shape caused by pregnancy can be addressed at the same time. Augmentation may not even be needed if breast size increases enough. Breast augmentation through an incision away from the nipple should not prevent breast feeding, though.

Wait.: If my patients are considering having pregnancies in the near future, i think that it is best for them to wait on breast surgery until afterwards. Breast-feeding is only one of the considerations involved. By waiting patients minimize the chances that additional surgery will be necessary after the changes that may occur during/after pregnancy.
