Orthopedic referral: If you've failed exercise therapy, the next step would be to see orthopedic specialist for more aggressive treatment. Depending on the nature of your problem and severity thereof, arthroscopic or more radical surgery could be offered or potentially other conservative treatment modalities.

See Orthopedist: If looseness is causing pain and instability then treatments are customized for each individual based on symptoms. Generally nonoperative mangement is advised such as therapy with emphasis on strengthening the shoulder gridle muscles. However, if symptoms becomes unbearable/painful or if you had history of injury resulting in shoulder being loose then surgery may be advised.
