Forceful vomiting: Esophageal rupture is not very common, but can be a very big deal; can cause life threatening complications, ifection, etc. More common in forced vomiting, retching. Bulimics can be at increased risk; uncommonly can be a post procedure complication after medical/gi procedures such as passage of an ng tube, endoscopic manipulations.

Alcohol, vomiting: There are some risk factors. Alcohol abuse with vomiting & wretching is a significant risk factor. Anybody with advanced esophageal cancer is also at risk. Having swallowed a caustic chemical (acid or base) can increase your risk, as well as having swallowed a sharp object.
