Many things: Should see a physician as there can be multiple causes. Heart issues can cause swelling but also venous insufficiency can cause it. See a physician who can do an appropriate work up.

Swelling: Commonest cause is venous insufficiency in the lower extremities. Other causes are kidneys, proteins, CHF , drugs and more.

See a doctor: You could have venous problems or heart problems causing your swelling. See a vascular doctor or you primary care doctor. Podiatrists can treat some swelling issues as well. This could be a symptom of a serious health problem.
