Potentially Serious : I would strongly recommend you have your boyfriend seek medical attention immediately. I am not an alarmist, but severe headaches associated with spitting up blood can be signs of very serious, even life-threatening problems. Again, i highly encourage you to seek medical attention--with you personal doctor or emergency room--as soon as possible.
