Usual culprits: Family history (bad genes), smoking, high blood pressure, high cholesterol, overweight, lack of exercise and unhealthy diet are the usual culprits.

Yes: There are controllable, and uncontrollable factors. The things that you can control are not smoking, watching cholesterol, controlling blood pressure and sugar levels. You cannot control your gender, age and genetics.
