Several: Gonorrhea or chlamydia of the throat or rectum won't be picked up by blood or urine samples. Throat/rectal swabs are required. Herpes is not diagnosed by blood or urine tests either. (blood tests will tell you whether you have herpes in your body, but they won't tell you whether you have an active outbreak.) there are other less common stds that require other tests as well.
