How do you know the difference between paranoid and just not knowing what the person is thinking?

Paranoid: "paranoid" means being suspicious of others' motives, such as thinking they mean harm to you. These fears are irrational and extreme -- beyond initial, expected cautiousness in getting to know new people. Someone who is paranoid would likely be reluctant to let others know what s/he is thinking.
