Many things: first and foremost it causes a very significant physical dependency, with a withdrawal syndrome complete with anxiety, irritability, headache, poor sleep and intense craving. It is also a cardiac stimulant and over time is partly responsible for the increase in arteriosclerotic heart disease (and other vascular disease) found in smokers and users of smokeless tobacco&e-cigs as well.
