Yes: Nasal steroid sprays may well inflame the tissue lining the perforation. This can lead to bleeding and irritation. For my patients with perforations i generally avoid nasal steroid sprays using oral medications instead. Have you seen an ENT and discussed either closure of the perforation or the placement of a septal button?

Nasal spray: be sure to spray it onto your turbinates and avoid spraying your septum. Ask your doctor to instruct you in proper use of the medication.
