Varies: A common form of sacral dimple is the tip of the tailbone is in contact with the skin. That type tends to be benign, but can fill with debris and hair and get infected.

In general, not: If you have always had a dimple, it probably is not an issue unless it gets repeatedly infected. This is different from a pilonidal sinus which needs attention.
