Complicated: If you do not have any weakness, bowel, or bladder dysfunction you may be able to be treated with manipulation, physical therapy, traction, and steroids ( either oral or epidural injections). If you have weakness or signs of neuro compromise surgery may be your best option.

Not necessarily: These are fairly common findings on image studies and do not necessarily mean that surgery is necessary. Whether you proceed to surgery depends on the severity of your symptoms and how you respond to medical management including physical therapy. Outcomes research has demonstrated that many patients, even with symptoms and image findings like you have, respond well to conservative management.
