Tried medicine on and off but it wont help. do you have any suggestions to get rid of acne and scars permanently or any home remedies?

Isotretinoin: You've got scars, and you've had failures with other medications. You're a grown man now and your acne is a major personal and professional liability. Acne is not curable but needs to be managed until it eventually remits on its own. You are probably a candidate for isotretinoin; be brave, ignore the scare stuff online, and go for it. You'll need microsurgery / laser / fillers for the scars.
