No.: The hymen is a thin membrane at the vagina's opening. The hymen has a hole in the middle that blood can go through, even before the first time a woman has intercourse. Women with "imperforate" hymen (without a hole in the membrane) may have a gush of blood when the hymen membrane is broken.

Not necessarily: Very variable, some hymens are so narrow that they do not tear with intercourse. A hymen can be torn with out any blood loss, with only loss of a drop or 2 of blood, with a short and minor trickle of blood and occasionally a gush. Latter is not that common.
