Not OK: It calls "lethargy". It is serious when combined with a high fever. Take her to the doctor.

Cannot tell: A thorough history is critical including duration of fever, associated symptoms, and exposure to illness. It would be important to know the childs' past medical history as well. She probably should be examined.
