Failure of therapy: Varies based on the patient. "co-morbid" conditions, or other medical illnesses that make the infection more concerning. Diabetes, cancer, chemotherapy, advanced HIV disease, or other things that modify your immune system can prompt an admission. Additionally, general health and response to therapy are important as well. If you failed outpatient treatment, that would prompt admission.
