Not necessary : At 21y/o without risk factor i doubt it's a heart problem. Possibly muscle pain. But you should consult your doctor to find out.

No: Arm pain can be from anything in the arm. Heart pain can radiate to the arm but the arm is not restricted like arthritis etc!
