Angina, cold airway?: Chest tightening on exertion, worse in the cold, is a classic presentation of angina pectoris which literally means a discomfort in the chest. It is the symptom of coronary narrowings (cad). On the other hand some folks get burning in their airways or even spasm of the airways on exercise (cold-induced asthma or a spectrum of same), due to airway reactivity in the cold. A stress test will help.
