See a doctor: For constipation that bad she may need to be seen. Lactulose is pretty powerful...If she is still constipated with that she may have an ileus (stopped bowels). Especially if her belly is poking out or she is having pain, she should be seen soon.

Lactulose is strong: Sounds like shes laxative dependent. Try miralax, (polyethylene glycol) you may have to give her multiple doses. If that doesnt work, give her a fleets enema. But ultimately, you have to increase her fiber intake. This can turn into a vicious cycle.

Try enemas.: Try a full enema ( soap suds or saline solution) through rectum. Contact the gastroenterologist or pcp. Needs rectal exam to rule out stool impaction requiring digital extraction of the stool.
