Sometimes kidney: If there is obstruction to fluid coming out of kidney there maybe less drainage when lying down. In erect position because of gravity kidneys drain more effectively.This may depend on how long in lying down position. This can be assessed by seeing doctor and imaging with ultrasound or nuclear study.Other musculoskeletal causes of pain in this area should be excluded.
