Involuntary screeching or shouting suggests Tourette syndrome -- although classical Tourette usually begins in childhood. Probably neither this nor your difficulty with some words is related to your trigeminal neuralgia, but it might be possible. Discuss with your doctor or, preferably, see a neurologist. Good luck.

If your paroxysmal symptoms are occurring during your attacks of facial pain, then the "screeching", and difficulty expressing yourself would indeed be a consequence of the trigeminal neuralgia. If these are isolated episodes, there needs to be a further evaluation as to a potential co-morbidity. A second opinion may be necessitated.
