Watch heart rate : Your doctor will just have to watch the baby's heart rate carefully in labor. A knot can decrease blood supply amd oxygen to the baby and increase the risk of fetal distress.

Need monitoring: True knots in the cord are not uncommon & thought to reflect the fetal activity around 12 wks when they have more room to flip around. As long as it is loose, babies normal heart rate will reflect good blood flow in the placenta.If the monitored heart rate shows problems the knot may be too tight & surgical delivery needed. I've seen as many as 3 knots in a kid that did well.

Sudden fetal death.: Although rare, some fetal deaths are the result of cord accidents, including true knots that are tied too tight for adequate fetal perfusion. Monitor the movements of your baby closely until delivery and get fetal surveillance at your obstetrician's office 2-3 times/week until delivery by 39 weeks. Your risk for cesarean section is increased, but normal delivery is still very likely. Best of luck!
