Condoms are highly effective in preventing STDs transmitted by genital fluids (gonorrhea, chlamydia, HIV and others) but somewhat less effective against those transmitted by skin-to-skin contact (herpes, HPV, syphilils). "White spots" on the condom do not suggest any higher risk than otherwise.
