As with any "food" material with significant nutrients, refrigeration after an hour or so will inhibit bacterial growth in the breast milk.

The process of extracting/expressing breast milk exposes it to skin germs that will make the possibility it will go bad a problem. If not consumed immediately or within the half hour I would toss remaining milk. If refrigerated immediately it can remain good for the day.
