How common is it for oral gonorrhoea to spread through the body (without new exposures) and become penile gonorrhoea?

It doesn't. If a person acquires penile gonorrhea, it was from sexual exposure to a carrier. If gonorrhea travels in the bloodstream (which is rare) it goes to joints/the heart/ the spinal fluid triggering meningitis. It does not selectively seek out the surface tissues of the genital tract.
