Had procainamide challenge to rule out brugada syndrome (history of vtach, family history of sca) negative for brugada but during the challenge, qt interval prolonged to 485 ms from baseline of 435 ms. significant ? could this be genetic long qt?

Were the QT intervals corrected for the rate ( QTc intervals)? Procainamide or its metabolite can prolong the QTc interval, A QTc of over 450ms could be significant. Genetic testing doesn't detect all congenital prolonged QT cases . Your cardiologist electrophysiologist is best to advise you however.
