Yes, have him evaluated at ER. Dehydration and electrolyte imbalance are suspected.

Most likely he is dehydrated. Encourage fluids including Pedialyte. Your son should be evaluated. A Pediatric acute care setting including the ED is appropriate. Watch for signs of fever, vomiting and fewer wet diapers.
