No risk, no worries. First, HIV isn't transmitted this way; not enough virus can survive on a speculum for more than a few minutes. And you can be pretty certain the doctor's office appropriately cleans their vaginal speculums after use; or, more likely, that they use single-use, disposable speculums. Don't worry about this.
