Starting oral bcp's "a week after period ended" may allow ovulation and conception to occur. What you passed during the "placebo" phase may have been an early "miscarriage". See your GYN physician for an exam. Oral bcp's should begin on the 2nd or 3rd day of an expected period to obviate possible ovulation.
