Anxiety. You are not alert enough to detect such changes in a sleep state, If you are aware, you are emerging from sleep and this expresses an anxiety state. Get checked out by your doc for starters. Then when your health is assured, find a life coach that can teach you stress management skills. In the process you may uncover some issues that create this process.
