It seems you have chronic lung disease -- maybe COPD (emphysema and/or chronic bronchitis) from smoking, maybe asthma, maybe both. After someone stops smoking, it takes at least 5 years for maximum improvement in bronchial or lung function. This problem needs close ongoing medical attention, perhaps from a pulmonary specialist.
