The care of your penis is not secondary to tight underwear but may be secondary to a disease called Peyronie’s disease which is a black formation in the shaft of the penis this may happen as a result of trauma in the past and the black make it larger my suggestion at this point would be to see urologist to evaluate the curvature in and evaluate for possible treatment
