Anal sphincterotomy, properly performed, is highly successful in the treatment of anal fissures. It is said that the surgery is less painful than the fissure itself. Most importantly, you'll have to be examined, advised and ,if advisable,operated upon by an experienced proctologist or colorectal surgeon before committing to surgery.
