Such a complex opinion is absolutely impossible to do on this site. Surgical indications will depend upon presence of compression, potential local damage, correlation to clinical progression. Headache, and limb pain would not necessarily be reasons for surgery. Consult with a neurosurgeon directly and obtain an opinion.
