Bell-clapper testicle is both transverse lying & hangs by spermatic cord within the the tunica vaginalis so that it is free to rotate. The description was because the author thought of testicle being similar to the clapper (clanger) of a bell which is only attached at the top end & tunica vaginalis was similar the bell surrounding the clapper. Similarity ends there as clapper swings not rotates
