Absloutely: You could have a fracture and it feel like a sprain.

Basilar thumb pain: Go to orthopedic surgeon and get xrays and exam. Check for scaphoid fracture, bennett fracture, gamekeeper's thumb, sprain, arthritis, distal radius fracture, among others.
