Ovaries perking up: Some women can have some ovarian activity years after they stop their period. If you have any bleeding at all though you should get an endometrial biopsy. Your intestines can cramp very much like period cramps. You may need a colonoscopy depending on your age.

No: Als is a progressive neuromuscular disorder. Only the motor neurons and muscles are affected. The sensory nerves remain intact, so the patient can feel pain, but the disease itself does not cause pain.

Lucky it's not: Good question. I don't think there is a simple answer for this. Possibly the irritated trigeminal nerve fatigues or is suppressed by the brain. A similar situation might be epilepsy where the damaged area only gets triggered periodically. Most people with tn couldn't stand having the pain constantly.

The issues is that the pain occurs at random and is unpredictable. The pain is often described as lancinating but can be severe and associated great emotional distress. There have been suicides related to it. Often the cause is a blood vessel causing compression of the nerve and a simple decompression procedure may help improve the pain.
