Doctor can evaluate: Fever with a "very sore" stiff neck has a reason. If the patient doesn't know of a non-worrisome reason, then he can see his primary care doctor if feeling "mild" or see the ER if "moderate" or "severe" symptoms. The ER can assess to see if there is an infection such as meningitis. Meningitis is rare, but very serious, so that is why an evaluation is needed as soon as possible.
