Heart failure: Because even with treatment, which make it "compensated" the heart has an overload of blood to pump which makes the pressure increase and build up through the lungs into the right heart. Depending on the mechanism of the damage that the heart received to cause the heart failure, it is also possible that there is a problem with the conduction system in the right heart.
